Pennsylvania State Immunization Requirements

* Take this list to your children's primary care physician *

NOTE: Most private insurance companies will not cover routine medical exams and immunizations. We recommend that you have your child immunized prior to coming to Carlisle.

Proof of Immunizations: (Defined as written verification of the day/month/year of administration, IN ENGLISH, of each dose.) NO STUDENT WILL BE PERMITTED TO ATTEND CLASSES UNTIL IMMUNIZATION RECORDS ARE COMPLETE. Hand carry your child's school records with you when you travel here; do not pack them with household items or clothing. They must be available to school personnel when you register. Have these records translated INTO ENGLISH.

Physical and Dental Examinations: All students newly arrived in the U.S. must have a medical exam. All students entering Grades 1, 3, or 7 must have had a dental exam within the past year.

Testing for tuberculosis for those from at-risk areas, MUST be administered in the US. IFP Staffers will assist you in getting the TB test in Dunham Clinic at Carlisle Barracks for your school aged children.

Children in ALL grades (K-12) require the following for attendance to any public school:

On the first day of school, unless the child has a medical or religious/philosophical exemption, a child must have had at least one dose of the above vaccinations or risk exclusion.
If a child does not have all the doses listed above, needs additional doses, and the next dose is medically appropriate, the child must receive that dose within the first five days of school or risk exclusion.
If a child does not have all the doses listed above, needs additional doses, and the next dose is not medical appropriate, the child must provide a medical plan within the first five days of school for obtaining the required immunizations or risk exclusion.
